Camp Independence Acknowledgement and Release

We are glad you have chosen to attend Camp Independence this year! Camp life offers many unique opportunities and experiences for you and your fellow campers. We hope you make new friends, learn a lot, and have a great time! At Camp Independence we expect campers to encourage, support, and show respect toward one another. Each person at camp has a responsibility to make camp life positive and enjoyable. We expect all campers to follow the behavior expectations outlined below. 1. I give permission for my child to take part in all camp activities. In consideration of the benefits derived from my child's attendance at Camp Independence, I expressly waive all claims against the camp and its staff, the Oklahoma Hemophilia Foundation, its officers, trustees, agents and employees on account of any accident, injury and/or illness that may occur to my child during camp. 2. I understand that it is against camp rules to be involved with tobacco in any form, alcohol use, illegal drugs or drugs which I do not have a prescription. I will not violate these rules by showing images of these items or using these items over virtual camp channels. In addition, I will not bring/display any weapons or pocketknives to virtual camp or use any of these items. 3. I understand that my child may be asked to no longer participate in the virtual group activities provided by Camp Independence if respectful and adequate online behavior cannot be maintained by the camper. 4. I understand that if a staff member or another camper feels that I am a danger to myself or anyone else, because of my behavior or something that I have said, I will be required to talk to the camp director and/or camp social worker. I understand that if the Camp Independence staff feels that it is necessary for my safety or the safety of others, I may be unable to participate in group virtual activities. 5. I understand that everyone at Camp Independence needs to be treated with respect and I will show respect for other's personal belongings, privacy and feelings. Camper Code of Conduct

Consequences of Inappropriate Behavior. The camp staff may take the following steps in instances of inappropriate behavior: 1. I understand that my camp counselor or the camp director may discuss the behavior with me. 2. I understand that a counselor or staff member may complete a written warning form describing the behavior that is not acceptable and will not be tolerated. The warning will outline other steps that may be taken by the camp staff and will be signed by the camp director and me. 3. I may be given a "time out" or not allowed to participate in an upcoming activity or activities. 4. Depending on the severity of the situation I may have to have a web or phone call with the camp director and/or camp social worker and/or counselor to come up with my own personal behavior contract, specifying what I must do to correct the behavior. 5. The camp director and/or camp social worker may call my parents to discuss the behavior with them and future consequences should the behavior continue. 6. I understand that I may be "sent home" from virtual camp immediately and possibly not be allowed to return to Camp Independence, with or without a behavior plan in place, even if it is my first occurrence of not following any of the behavior expectations. Some behaviors that may result in immediately being sent home from camp include, but are not limited to: acts of racism, stealing, sexual harassment or sexual misconduct, causing or attempting to cause physical harm to others, and bullying, in any form, whether it be physical or verbally teasing at another's expense, gossiping or excluding others at any time for any reason. I have read, understand, and will fully comply with the Camp Independence Behavior Expectations. I have also read, understand, and fully agree to the Consequences of Inappropriate Behavior as part of the Camper Code of Conduct. *
